The Crash Records Center’s Fatals Unit has recently launched an online version of the MV-104EN.

The form is now fillable online and in addition to the traditional filing via fax the form can now be emailed directly to DMV using the email address This email address is being protected from spambots. You need JavaScript enabled to view it..

Moving forward the MV-104EN will ONLY be available online. https://dmv.ny.gov/forms/mv104en.pdf
